The following forms are offered through the Office of Student Involvement and Leadership. All the forms can be submitted electronically using the links below. In order to host an event, chalk, put up large signs on-campus or promote in the windows at the University Center desk an organization must be registered with SIL. Additionally, only registered organizations will have access to the equipment that is available for check out through the SIL office.
Organization Registration Forms
Student Organization Registration Form
All organizations on campus are required to submit an organization registration form at the beginning of each school year. Submission of this form is necessary for an organization to be recognized as an official student organization at the University of New Orleans and allows them to gain access to the services offered through SIL.
Every student organization is required to have a constitution. New student organizations will need to submit their constitution before they can be considered an official registered organization. Organizations continuing from previous years who already have constitutions on file will not need to re-submit these constitutions for re-registration. This example constitution can be used as a model while creating a new constitution.
Change of Officer Form
If there is a change of an officer anytime throughout the school year, please fill out this form to update SIL of the new positions.
Event Registration Form
In order to host an event on campus, a student organization must turn in this event registration form for approval. Remember: A student organization must be registered with the Office of Student Involvement and Leadership to host events on campus.
Organization Equipment Check-Out Form
The Office of Student Involvement and Leadership offers equipment for student organizations to use for on-campus events. If an organization is interested in checking out equipment through SIL, they must complete the organization check-out form and return it to the office at least one week prior to the event.
Chalking Registration Form
If an organization would like to promote by chalking on the sidewalks, they must turn in the chalking registration form to SIL.
Window Registration Form
Window spaces in the University Center Deck may be reserved by student organizations for a span of one week at a time. If an organization is interested on using the windows for promotion, they must complete and turn in the window registration form to SIL for approval. **Students interested in posting banners in the Atrium must receive approval from the Ask Lafitte Desk.
Each organization is granted two (2) 24"x36" posters each semester. To request your posters, please email firstname.lastname@example.org with the following:
- Date posters need to be printed by
- Organization name
- A high resolution image file (.pdf, .jpg, .png)
The Office of Student Involvement & Leadership reserves the right not to print posters that do not meet the resolution needed to print at poster size or that violate university policy. SIL will not edit image files.
